When you create your questions, ask your self "what am I going to do with this information?", if you do not know, don't ask the question.
Contact a few of your users (3 - 5) with a link directly to the feedback page, then ask them for feedback on your questions.
Keep the questions short and simple. A clear question is quicker to answer.
It's not easy to come up with good questions, that's why we provide some base questions to select from if you are unsure.
